What Human Trials Have Shown

NMN does raise blood NAD+ levels in humans. This is well-established. A 2022 RCT in Nature Aging found that 250mg and 500mg of NMN daily for 12 weeks significantly raised blood NAD+ concentrations. NR (nicotinamide riboside) shows similar effects. The question is whether raised blood NAD+ actually translates to meaningful cellular effects.

Muscle NAD+ is harder to raise than blood NAD+. A critical 2023 study found that despite significant blood NAD+ increases with supplementation, skeletal muscle NAD+ levels didn’t change. Based on current evidence, the men most likely to see meaningful benefit from NAD+ supplementation:

  • Men over 50 with declining energy, muscle function, or metabolic efficiency
  • Men with metabolic syndrome or type 2 diabetes (some trial evidence for insulin sensitivity improvement)
  • Men who drink regularly (alcohol depletes NAD+)
  • Men doing intensive endurance training (NAD+ is consumed in mitochondrial energy production at high intensities)

Men who are unlikely to see dramatic effects:

  • Healthy men under 40 with adequate sleep, low alcohol use, and regular exercise — NAD+ metabolism is likely already well-supported by lifestyle

Either way: sleep, exercise, and alcohol reduction will do more for your cellular NAD+ metabolism than any supplement — because they regulate the enzymes that consume and synthesize NAD+ endogenously. The supplement market’s interest in NAD+ is real. But the fundamentals still matter more.
