You’ve been tired for two years. Not “I need a nap” tired — the kind of tired that sleep doesn’t fix. You’ve noticed the brain fog, the stubborn weight around your middle, the workouts that take longer to recover from. You chalked it all up to being busy, getting older, not sleeping enough.
Maybe. But there’s another explanation your annual physical almost certainly missed: chronic low-grade inflammation. And the research connecting it to nearly everything you’re experiencing is harder to ignore the more you look at it.
What Chronic Inflammation Actually Is
Here’s where it gets interesting.
Acute inflammation is your immune system doing its job — swelling a sprained ankle, sending white blood cells to fight an infection. It starts, it resolves, it’s done.
Chronic low-grade inflammation is different. It’s your immune system stuck in a low-level activation state, continuously releasing pro-inflammatory cytokines (IL-6, TNF-α, IL-1β) without a clear threat to resolve against. Think of it as your body’s alarm system ringing at 10% volume, 24 hours a day. You stop noticing the sound. But the system is still burning resources responding to it.
The downstream effects touch nearly every system in your body.

This is the part most articles skip. Three reasons men miss chronic inflammation:
1. The symptoms are vague and overlap with “being busy.” Fatigue. Brain fog. Low motivation. Mild joint stiffness. Slow recovery from workouts. Weight that won’t budge despite effort. Every one of these has a dozen possible explanations, so most men — and most doctors — attribute them to lifestyle before looking deeper.
2. Standard bloodwork doesn’t catch it. A basic metabolic panel and CBC won’t show systemic inflammation. You need hs-CRP (high-sensitivity C-reactive protein) at minimum, and ideally homocysteine, ferritin, and fasting insulin alongside it. Most annual physicals don’t include these unless you ask.
3. The medical system treats downstream effects, not root causes. Depression? Here’s an SSRI. Joint pain? Try ibuprofen. Can’t lose weight? Eat less. Each symptom gets addressed in isolation when the upstream driver — chronic inflammatory signaling — may be the same for all of them.

Depression and mood disorders: A 2019 meta-analysis in JAMA Psychiatry by Osimo and colleagues found that patients with depression had significantly elevated levels of CRP, IL-6, and TNF-α compared to healthy controls. The “inflammatory hypothesis of depression” is now a major research frontier — meaning there’s a real possibility that treating inflammation could treat depression.
Cardiovascular disease: The CANTOS trial, published in the New England Journal of Medicine in 2017 by Ridker and colleagues, demonstrated that reducing inflammation with canakinumab — an IL-1β inhibitor — reduced cardiovascular events independent of cholesterol levels. This was a landmark moment: proof that inflammation drives heart disease separately from lipids.
Cognitive decline and brain fog: Marsland and colleagues published findings in Brain, Behavior, and Immunity in 2015 showing that elevated inflammatory markers are associated with reduced processing speed, poorer working memory, and accelerated cognitive aging. If your thinking feels slower than it used to, this is worth paying attention to.
Insulin resistance and metabolic dysfunction: Hotamisligil showed in Nature in 2006 that chronic inflammation impairs insulin signaling at the cellular level. TNF-α directly interferes with insulin receptor substrate-1, creating a feedback loop: inflammation leads to insulin resistance, which leads to more visceral fat, which creates more inflammation.
Testosterone suppression: Pro-inflammatory cytokines inhibit GnRH pulsatility and Leydig cell function, directly suppressing testosterone production — as Tremellen documented in 2016. If your testosterone is low and your inflammation is high, the inflammation may be the upstream cause, not the downstream effect.
The Five Major Drivers
Here’s where it gets useful.
Research points to five modifiable factors that sustain chronic inflammation in otherwise “healthy” men.
1. Visceral Fat
Visceral adipose tissue isn’t inert storage — it’s an active endocrine organ that secretes IL-6, TNF-α, and leptin. Desprès showed in 2012 that a waist circumference above 40 inches in men is independently associated with elevated inflammatory markers regardless of BMI. What to do: prioritize waist circumference over scale weight. Resistance training combined with a caloric deficit targets visceral fat more effectively than cardio alone.
2. Poor Sleep
Even partial sleep restriction measurably elevates inflammatory markers, as documented in Biological Psychiatry in 2016. Chronic poor sleep maintains this elevation indefinitely. What to do: non-negotiable 7+ hours. Consistent wake time matters more than bedtime. Dark, cool room. No screens 60 minutes before bed.
3. Ultra-Processed Diet
Diets high in refined carbohydrates, seed oils, and ultra-processed foods are consistently associated with elevated inflammatory markers. The PREDIMED trial showed that a Mediterranean diet reduced hs-CRP by 26% over 5 years compared to a low-fat control diet — that’s Estruch and colleagues in the NEJM in 2018. The specific diet label doesn’t matter. What matters: high omega-3, high fiber (30g+ per day), high polyphenols, low ultra-processed food.
4. Sedentary Behavior
Regular moderate exercise produces anti-inflammatory myokines (IL-10, IL-1ra) that actively counter chronic inflammation. Fedewa and colleagues showed in Medicine and Science in Sports and Exercise in 2017 that 150 minutes per week of moderate activity reduces CRP by approximately 20–30%. The dose-response is clear. Consistency beats intensity.
5. Chronic Psychological Stress
The HPA axis and immune system are bidirectionally linked. Chronic stress eventually leads to glucocorticoid resistance, where immune cells stop responding to cortisol’s anti-inflammatory signal. Cohen and colleagues documented this in PNAS in 2012. The result: unregulated inflammatory activity. Stress management isn’t soft — it’s immunology. The interventions with the strongest anti-inflammatory evidence: regular exercise, adequate sleep, and mindfulness-based stress reduction.
How to Measure It
Here’s where it gets interesting.

Minimum panel:
- hs-CRP: Under 1.0 mg/L is optimal. Under 3.0 is “normal.” Most men should aim for under 1.
- Fasting insulin: Under 8 μIU/mL is optimal. Elevated fasting insulin is an early inflammation-adjacent marker.
Better panel (add these):
- Homocysteine: Under 9 μmol/L. Elevated levels indicate B-vitamin issues and cardiovascular inflammation risk.
- Ferritin: 50–150 ng/mL is the sweet spot. Over 200 with elevated CRP warrants investigation.
- Omega-3 Index: Over 8%. Under 4% means you’re starting from a pro-inflammatory baseline.
Most of these are cheap, standard CLIA tests. Your doctor can order them. If they won’t, direct-to-consumer lab services will.
The 90-Day Protocol the Research Supports

Weeks 1–4 (Foundation): Lock in 7+ hours of sleep with a consistent wake time. Start 30 minutes of daily walking plus 2 resistance sessions per week. Eliminate the worst dietary offenders — sugary drinks, fried food, excessive alcohol. Get baseline labs: hs-CRP, fasting insulin, vitamin D at minimum.
Weeks 5–8 (Optimization): Add 2–3 servings of fatty fish per week, 30g of fiber daily, and daily greens. Progress to 150+ minutes per week of zone 2 cardio plus 3 resistance sessions. If labs show deficiencies: vitamin D3 (2,000–4,000 IU/day), omega-3 (2g EPA+DHA daily), magnesium glycinate (300–400mg daily). Pick one evidence-backed stress practice — 10 minutes daily of meditation, breathwork, or structured journaling.
Weeks 9–12 (Verification): Retest hs-CRP, fasting insulin, and any flagged markers. Assess energy, mood, focus, recovery, and body composition. Adjust based on data, not feelings. Most men who follow this pattern see measurable CRP reduction within 90 days. The research supports this timeline.
